Tenby's all-weather lifeboat Haydn Miller was requested to launch at 9 am on Tuesday after a report that a resident of Caldey Island had fallen and had a suspected ankle fracture.
The lifeboat launched with a doctor to assess the casualty and also some of Tenby's Coastguard Rescue Team aboard in case a helicopter landing site needed to be set-up.
Once at Caldey, the Y boat was launched as there was too much swell on the slipway to get the lifeboat alongside.
The Y boat ferried the doctor and coastguards ashore. The casualty's leg was immobilised and he was then brought down to the slipway and taken aboard the lifeboat via the Y boat.
Once everyone was back aboard, the casualty was taken back to Tenby and was met in the harbour by paramedics.
