A blueprint for the future of health and social care in Pembrokeshire was launched last week.
The joint plan between Pembrokeshire Local Health Board and Pembrokeshire County Council aims to target key health and social care issues in the county.
People are being urged to have their say in how and where money should be spent to improve services in Pembrokeshire.
The draft document has highlighted seven main areas, which health and county council chiefs want to target, including obesity, diabetes and dental health.
Local Health Board chairman, Chris Martin, urged people to get involved and make their views known.
He said: "The Health, Social Care and Well-Being Strategy sets out how quality of life for people in Pembrokeshire can be improved and how certain key services can be developed. I hope people will take this opportunity to tell us their priorities."
He added that major changes were needed to the way health and social care services are delivered in Wales.
He said: "Professionals who deliver health and social care services need to look again at how they operate and ask if there are better ways of working.
"Increasing the number of hospital beds is not the answer - many services can be provided as well, if not better, in the community and people prefer to stay out of hospital if they can.
"The pressure on services can be reduced by focusing on improving quality of life via the prevention of illness and identifying and treating illnesses early. We want people to take responsibility for their own health and we hope this document will help us meet those targets."
The Health Board, County Council and other key agencies are organising a series of Health Challenge Road Shows this summer when people will have the chance to comment on the document.
The leader of Pembrokeshire County Council, Clr. Maurice Hughes, said it was important to seize the moment.
He said: "Now is the time for action. Currently there is a unique conjunction of factors, which can accomplish better performance and essential reform of our health and social care services.
"However, failure to grasp this opportunity will mean continuing unsustainability in health services and serious consequences for confidence in other public services."
