Pembrokeshire County Council's 'Team Around the Child' service scooped an award at the prestigious Excellence Wales Awards in Llandudno last Thursday.
Organised by the Welsh Local Government Association, the awards are an opportunity for councils and their partners to demonstrate how good their services are and to share their good practice with other areas.
The 'Team Around the Child' service won a Commended award for its work to create a single point of referral for young people with additional needs.
Following a referral, a trained 'Teams Around the Child' (TAC) officer calls together a group of support workers from across the council and other organisations, such as the health service, with the young person and their family to create a co-ordinated action plan to meet the young person's needs.
The TAC officer then follows up on the progress of the young person to make sure that their needs are being met.
The development of the 'Teams Around the Child' project was overseen by the Pembrokeshire Children and Young People's partnership whose good work was itself recognised in a recent Esytn inspection.
This multi-agency approach means a much better co-ordinated set of services.
County Clr. Huw George, cabinet member for children, young people, learning, said: "I am very pleased that the achievements of the council and its partners - especially the Hywel Dda NHS Trust - have been recognised.
"The team has only been operational since January 2009 and I am confident that it will make a real difference to young people's lives."
Pembrokeshire County Council also picked up a Certificate of Recognition awarded where an authority demonstrates pockets of exemplary practice worthy of recognition.
Pembrokeshire collaborated with Neath-Port Talbot and Carmarthenshire County Council in procuring a joint Library Management System.
The new system has already saved £50,000, with an additional £20,00 saved over the next five years.
